What is Medication Therapy Management (MTM) and how can it help me?
 

The MTM program is highly encouraged for members with multiple health conditions taking a variety of medications. In this program, we’ll work to ensure there are no negative interactions between your prescriptions. But that’s just the beginning. We’ll also make sure your medications work well together for the best possible health outcomes. And we’ll help you get the most cost-effective medications.

 

The program begins with a Comprehensive Medication Review (CMR). If you qualify, one of our licensed pharmacists will call you in the privacy of your own home, at no extra cost, to walk you through it.

What can I expect as a member?

A Comprehensive Medication Review (CMR)
 

  •  One of our licensed pharmacists will review your medications with you.
  •  The review takes place over the phone, at your convenience, so you can discuss your questions in detail and get answers.
  •  You’ll receive a detailed summary of your review, including any action plans and further guidance.

Ongoing medication management

 

  •   Your medications will automatically be reviewed at least once every three months.
  •   You’ll be contacted to confirm your medications.
  •   If there are recommendations for changes in your medications, you or your doctor will be contacted.
  •   You can also sign up for refill reminders, home delivery, and extended-day supplies.

How do I qualify for the MTM program?

You have three or more chronic health conditions including congestive heart failure, diabetes, high blood pressure, and others.

You take eight or more of certain medications covered by Medicare Part D.

You are spending $4,696 or more a year on Part D covered medications.
