Take your Health Risk Assessment

A Health Risk Assessment (HRA) is a no-cost, confidential online tool designed to help you better understand your overall health. The HRA asks you a series of questions about your:

  • Health history
  • Current health
  • Daily routine

The information from your HRA helps your doctor provide the right kind of care for you. It also provides you with specific resources that can help you live healthier. 

There is no cost for plan members.

Your HRA should be taken before you take your annual checkup, so you can print it out and bring it with you to the doctor. The entire test takes about ten minutes to complete.
